Survey (Lean/Six Sigma)

Dear professional engineers,

I am an engineer currently studying for a PhD in manufacturing, my reserach is in the lean six sigma area. For this reason I need to collect some data via 2 questionaires about implementation of lean and six sigma in manufacturing companies. I can provide the evidences that this is an academic research and the data provided will be treated with high confidentiality.
