Continuouse Improvement vs Transformational change - whats the difference?

Can anyone provide guidance on the difference between continuous Improvement and transformational change. We are currently trying to pin down the distinction to add clarity to our Change programme are are looking at distinctions on a number of dimensions including: time to delivery of improvements, time to delivery of benefits; toolkit; degree of change, cost etc.

does anyone have any ideas on where the boundaries should lie or at least what the extreames of each of these dimensions is?
